Biological Function
HLA-DRA encodes the alpha chain of the MHC class II molecule HLA-DR, expressed on monocytes, macrophages, and dendritic cells. HLA-DR is essential for antigen presentation to CD4+ T cells — the initiation point of the adaptive immune response. Monocyte HLA-DR expression is the gold-standard flow cytometry marker for immunocompetence in critical care.

HLA-DRA downregulation is the transcriptomic signature of sepsis-induced immunoparalysis — the state where the immune system becomes unable to mount an effective response to secondary infections. Monitoring HLA-DRA mRNA provides a faster, more accessible readout of immune competence than flow cytometry, enabling earlier identification of immunosuppressed patients who need immunostimulatory therapy.
Expression Context
Constitutively expressed on antigen-presenting cells. Downregulated during sepsis, major surgery, and severe trauma (immune paralysis). Recovery of HLA-DR expression correlates with clinical improvement and reduced mortality.
